- One business letter from US Company
- Copy of driver’s license or passport of the person who signed the US business letter
- One letter of invitation from company in Thailand- must specify single or multiple entry
- Copy of Thai company registration certificate
- Copy of applicant’s most recent personal bank statement (minimum balance for single entry visa should be $1000 or $7000 for multiple entry visas)
- Copy of round trip Itinerary
- Copy of hotel reservation
- Copy of passport pages with travel stamps in the last year
- Copy of applicant’s driver’s license
- Photo of the applicant holding their US passport open to the information page
