What passport
service do you need?
Note: You must use this method for any one of the reasons listed below:
- I have had a previous US Passport issued to me and I am able to submit it with my order, and it is in Fair condition.
- The passport I am submitting is the most recently issued US Passport I have had issued.
- I turned 16 before this passport was issued.
* Government fee of $209.53 to be paid to the "U.S. Department of State" is not included.
US Passport Renewal RequirementsYour U.S Passport can only be renewed if ALL of the following conditions are met:
- You still have your U.S. Passport in your possession.
- Your previous U.S. Passport was issued when you were age 16 or over.
- Your previous U.S. Passport was issued no more than 15 years ago.
- Your U.S. Passport is undamaged.
Instructions
- Order Form
- Print 2 copies of a completed DS-82 passport application form.
- Two passport photos. 2″x 2″ white background.
- Proof of Identity. (a valid driver’s license or government issued photo I.D)
- Print 2 copies of a completed Letter of Authorization.
- Print 2 copies of your Travel itinerary, showing dates of travel.
- A check for $190.00 (For Adults) or $160.00 (For Minors under 16) and a check for $19.53. The checks should be payable to the "US Department of State", on the memo line, applicant DOB and full name is required.
